INDIVIDUALS - PRINT THIS FORM, then order the Achievement Badges you wish to earn by completing the above Order Form and returning it, along with your completed requirements and the required fee, to the address shown. If the Badge requirements are properly completed, your Achievement Badge will be mailed to you. Be sure to put your phone number and e-mail address on the order form in case we need to contact you.
ASHAYC's, 4-H CLUBS, RIDING CLUBS & OTHER BONAFIDE GROUPS WITH AN ADULT LEADER may order the Achievement badges you want for your club by printing and completing the above order form and returning it, along with the required fee, to the address shown and your badges will be sent to you. As the club's counselor, you may determine when each of your members has satisfactorily completed the requirements for a badge. It is not necessary for you to return the completed answer sheets to ASIYP. PLEASE NOTE that ASIYP offers a "Letter" program (similar to an athletic letter). Part of the requirement to earn this Letter is to complete a certain number of Achievement Badges.
Club leaders will also receive a Badge Completion Form. You must return the name, address and birthdate of each member who earns a badge, along with the name of the badge and the date the requirements were completed in order for the badge to count toward earning the ASIYP LETTER. Letters will be presented, to those who qualify, at no charge.
